Tennessee Excavation Company Fined for Trench Collapse That Killed 1, Injured 1

Trench2

Employers need to take appropriate steps to protect workers. When they fail to perform mandatory training and inspections, employees can suffer serious injuries or even fatalities in an accident.

Sadly, this happened in December. A trench collapse in Tennessee killed one person and injured another. The excavation company responsible for the accident was recently fined a whopping $210,000 for its negligence.

The fatal trench collapse occurred on December 11, 2025, at Big Ridge State Park. A crew was digging a trench for sewer lines serving the park’s ranger residence and cabins. According to the Tennessee Department of Labor and Workforce Development, the trench was approximately 15 feet deep and was dug in highly unstable “Type C” soil, which carries a significant risk of collapse.

According to the investigation, one worker was shoveling soil inside the trench when a wall suddenly collapsed, burying him. A second employee attempted to help, triggering another collapse. One worker died at the scene, while the surviving employee spent five hours trapped before being rescued with multiple fractures, bruised ribs and other injuries.

Investigators said the company did not use protective equipment such as trench boxes, despite having access to them. Instead, workers reportedly relied on spotters to watch for signs of a cave-in.  State investigators found that the company failed to properly train employees, inspect the worksite and provide adequate protective systems for workers inside the trench. The state described the company’s actions as a willful disregard for employee safety.

The company received three citations related to inadequate employee training, failure to inspect the trench and failure to use proper protective systems. State officials said the conditions at the site made the trench particularly dangerous and that the collapse could likely have been prevented with appropriate safety measures.

Trench collapses are among the most dangerous construction accidents because they happen suddenly and often leave workers with little or no time to escape. Workers typically have only seconds to react. Even experienced workers may be unable to climb out before the soil collapses around them. Even a relatively shallow trench can become deadly if the walls give way.

The greatest danger is being buried under thousands of pounds of soil. A single cubic yard of dirt can weigh as much as a car. When a trench collapses, workers may be pinned, crushed, or completely buried, leading to:

  • Suffocation
  • Broken bones
  • Internal organ damage
  • Spinal cord injuries
  • Traumatic brain injuries
  • Death
